Please use the form below to make your nominations which should include full contact details and a statement of no more than 500 words explaining why your nominees deserve to win.

The statement should give specific examples of the measures the organisation or person employs to fulfil the award criteria and examples of how and why they embody and embrace happiness in their professional capacity.

You can enter more than one organisation or person but please use separate forms for each nominee.

Please make sure you let nominees know you have nominated them so when we contact them they are aware of the award. finalists will be notified by email by 24th Oct 2016.
